Appointment & Cancellation Policy
- If you see that you are unable to keep your office visit appointment, we ask for 24 hours notice.
- If you need to reschedule your procedure / surgery we ask for 48 hours notice.
- Cancellation charges may apply if timely notice is not given.
Each appointment is valuable to our patients and physicians. We want to ensure we get everyone seen as promptly as possible.
Medical Records, Referrals & Fax
- Medical records request: complete the 2020 Medical Release (PDF) and return it to the office or fax it to (941) 391-6937. Requests normally take 7–10 business days.
- Referring offices: fax referrals and records to (941) 391-6937, or call (941) 391-5102.
